Activates during any task involving architectural, structural, or design decisions - choosing libraries, designing schemas, creating modules, making tradeoffs, selecting patterns. Also activates when DECISIONS.md already exists in the project. Do NOT activate for trivial changes like typos, renames, or formatting."
